The one who attack first on a 1vs1vs1 battle(Player vs Player vs Neutral) is random:

And yeah, as you've said, Both units attack. More accudately, one unit deal "attack" damage, and the other deal "defense" damage. If let's say both units have 1 HP and both units roll 5 attack/defense, the attacker will die first.

First: 
Who wins,thats easy,wins player that only has units at the end of battle for neutral. 
Second: 
One who is defending is who faster attacked that city,then player that attacked slower attacking and other player and neutral. 
3rd: 
Same as 2nd.
